Personal Wellbeing Plan

An editable document that can be used to plan how you will support your own wellbeing.  The visuals are based on two evidence-based models of wellbeing: Te Whare Tapa Whā Hauora model and the Positive Psychology PERMA model.  

Within the 'daily', 'weekly' and 'monthly' contexts of your life, plan activities that you enjoy and link them to each of the 9 aspects of these two models.  Press CTRL + Click (PC) or CMD + Click (Mac) to go to a website for each model that provides more information to support you with the activities you decide to put in your plan. 

For a balanced approach to your wellbeing, try to plan activities that link to all 9 aspects of both wellbeing models.  This plan could also be easily adapted for children by using different visuals to represent each aspect of the two wellbeing models.
