These might be useful if colleagues are conducting the review on paper as part of a discussion:

Suzie Vesper has developed templates to structure discussion..and says that these sheets have been designed as a way of recording progress against the e-Learning Planning Framework.  There are two templates:  

1. one for teachers to map their own use of e-learning as individuals: e-Learning Planning Framework | Individual
