Collecting student voice

Looking for some good questions /questionnaires that others have used to collect student voice in our MLE's. Also want my Student Leaders (Year 7-8) to be able to be involved in this collection - anyone that has created anything would be a great help. Thanks so much.

    Hi Lisa,

    We have recently used google forms to gather student voice. One on student engagement and one on how students feel about a change in our teaching practice reflecting MLP.

    Hi Tracey,
    I'd be keen to look please.
    I did a form for the parents of my class pre-term which was very useful - questions like 'How will I know if your child is unhappy / frustrated. What signs do I look for? How do we deal with this?
    Also a very good series of Google vids on YouTube about using Google forms...

    I would like to have a look at this too if you are happy to share.  My Teaching as Inquiry goal, this year is around MLP's and one of my ideas on my action plan is to get student voice.
